Health Physical & ECG Information
Starting in the 2026-27 school year, Volusia County Schools is requiring that ALL students participating in Marching Band at the high school level submit a clear health physical form (FHSAA - EL2 Preparticipation Physical Evaluation) and evidence of a clear ECG (FHSAA - EL1 Electrocardiogram Screening).
The EL2 physical must be completed at the beginning of every year (clear through April), but the ECG (EL1) is only required to be completed one time. Please only submit PAGE 4 of the EL2 form.
You can take care of these requirements through your primary care physician. You can also attend events sponsored by AdventHealth or "Who We Play For". Many of these events are free or minimal cost. To search for "Who We Play For" heart screening events, CLICK HERE.
We are asking that all band students complete these requirements by JULY 1.
Upcoming Health Physical & ECG Opportunities
Below are links for band students to register for ECG screenings, to be held in early June, for $20 per student (note that event this is ECG’s only, not health physicals). The priority is to attend the event at your HOME SCHOOL (DELAND HS), however, if you absolutely cannot attend that particular date/time, you can register for another location in order to complete their requirements.
Please make sure you take care of your health physical and ECG screening by July 1.

Important Forms
ALL band students are required to complete and submit the forms listed below. These forms must be completed and submitted BEFORE band camp. We are asking that all band students submit their completed forms no later than JULY 1.

How to Submit Forms
This year, all forms must be submitted electronically. This is so that we can clear as many students as possible before band camp starts. This also ensures that parents have a copy of the forms that are submitted for their own records and helps us have better access to completed forms when we are away from school.
